Community Health Needs Assessment

Apache County Public Health District and White Mountain Regional Medical Center have partnered to assess our community health needs. The survey helps both Apache County and White Mountain Regional Medical Center determine our community's health needs and strengths. The tool also helps us in planning for the future needs of our community. Please take the assessment through this link or in person at the health department.
